Rome Youth Hockey: Fundraising Approval Form

Please complete the fundraising approval form two weeks prior to the board meeting. 

The board meets the second week of each month. 

Board members must approve the fundraiser before any arrangements are made. 

Fundraising coordinators will update each team upon approval. 

The first 25% of total sales from the first fundraiser will be contributed to the associations general fund, not to exceed $250.00.
